3.Request for Maternity leave dates letter

Here you will find an example letter which you could issue once the pregnant employee has informed you of the Expected Week of Childbirth (EWC) . The letter will look to establish the date on which the employee wishes the maternity leave to commence along with details around what happens if the employee wishes to take Ordinary Maternity Leave (OML) and Additional Maternity Leave (AML)

The letter needs further personalisation as shown in RED.
