3. Flexible working request form

This form can be accessed and issued to an employee if they mention that they would like to make a formal request for flexible working. This form is based on the form found on Gov.UK so the employee could locate it themselves, however, accessing it here and issuing to the employee may just speed up the process for both sides. This is an alternative to the letter shown in this section – the employee (and indeed you) may find this a better template than a formal letter.

As with the employee letter; it helps by setting out the areas that the employee must consider if they wish to make a formal application
